The Additional Needs Hospital Passport
I put this together following poor care in an NHS hospital. Hospital Passports have been in use for people with learning disabilities for some time, yet to my knowledge there is no equivalent for people with physical disabilities, though some condition specific charities have made them for particular groups. This is my attempt to fill the gap, and it is not a recognised medical record. However, if your local hospital does not have an effective way of recording your needs, please feel free to use this template as a stop gap measure.
